Incident Investigation in the mining industry is a systematic, formal process conducted following any unplanned event — including injuries, near-misses, property damage, environmental releases, or process upsets — to identify the root causes, contributing factors, and systemic deficiencies that allowed the event to occur, with the ultimate goal of preventing recurrence. Modern incident investigation methodologies used across bauxite, gold, iron ore, and diamond mining operations include the 5-Why analysis, Bow-Tie analysis, Tripod-Beta, and the more comprehensive ICAM (Incident Cause Analysis Method). Investigations are conducted by a multidisciplinary team comprising the mine manager, safety professionals, technical experts, worker representatives, and sometimes external specialists. In bauxite mining, investigations often address ground disturbance, equipment rollovers, and dust-related health incidents. In gold mining, investigations focus on cyanide exposures, fall-of-ground in underground operations, and processing plant accidents. In iron ore operations, investigations into conveyor entanglements, rail incidents, and dust explosions are common. In diamond mining, security breaches and equipment failures during sorting are investigated. Findings are documented in investigation reports, corrective actions are assigned, and learnings are shared across the organisation and sometimes the wider industry.
